What is the process for a policyholder to get a predetermination letter or check if certain CPT codes will be covered?

Please be advised that if the insured is requesting a pre-determination, them or the provider office would need to email ACPredeterminations@manhattanlife.com or fax the request with the codes to 281-916-7471. The turnaround time for processing is 72 hours.

Please advise them to include the following:

1. Pre-D Requests must include all CPT and DX codes, as well as any applicable medical records.
2. Pre-D Requests must specify that the request is for a PRE-DETERMINATION, and NOT a Preauthorization or Precertification. If mislabeled, the document could be misrouted and delayed.
3. Please submit the pre-determination request to the attention of “Appeals Specialist”
4. Pre-D Requests must Include contact information (i.e., name and fax number) so that the response can be sent directly to the requestor.
