Under the DVH Select plan, when can a crown, inlay, or onlay be replaced?

 Under the DVH Select plan, crowns, inlays, and onlays are eligible for replacement benefits only after 5 years from the date of the original placement. Temporary inlays, temporary onlays, and prefabricated crowns that are more than one year old are considered permanent appliances and are subject to the 5-year replacement limitation.
