Under the DVH Select plan, when is an implant replacement covered?

Under the DVH Select plan, implant replacement is considered a covered charge only when:

• The existing implant was placed at least 5 years before the replacement, and
• The implant has failed.

If both conditions are met, replacement benefits may be available according to the terms of the plan.
